Training Request Form

Facilitators

In order to request support for an upcoming training, please completed the following form no later than 2 weeks prior to the training to ensure that your materials will be available and that participants will have time to register. Please also keep in mind the criteria below when requesting support.

Agencies/Organizations

Prior to completing the form below, please be sure to check the calendar for upcoming trainings near you.

The coalition provides two tiers of support to agenies/organizations who are interested in suicide prevention trainings. Full support includes a facilitator, training materials for participants and facilitators, promotion on training calendar and coalition social media platforms, and online registration. Partial support for agencies includes a facilitator, materials for the facilitator, promotion of training on calendar and coalition social media platforms, and online registrations. Agencies who receive partial support will be required to purchase training materials for participants. 

The Coalition provides full support to trainings that meet the following criteria:

• Audience (any one of the following)
  • Training is open to all community members at no cost
  • Training is being provided to students/youth at no cost
  • Training is being offered to veterans at no cost
• Facilitator has a current document of collaboration in place with the coalition
• Training follows coalition training protocol
  • Online registration
  • Use of online pre/post tests for data collection
  • Facilitator meets all fidelity requirements of curriculum being offered
  • No cost to participants

The Coalition provides partial support to trainings that meet the following criteria: 

• Audience
  • Training is open only to those who work for agency/organization hosting the training
• Facilitator has a current document of collaboration in place with the coalition
• Training follows coalition training protocol
  • Online registration
  • Use of online pre/post tests for data collection
  • Facilitator meets all fidelity requirements of curriculum being offered

Trainings will not be eligible for coalition support if:

• There is a cost for participants to attend the training
• There is not a current Document of Collaboration in place
• Does not follow the coalition training protocol
